摘要:
目的 分析高强度聚焦超声(HIFU)联合清宫术治疗剖宫产瘢痕妊娠的疗效。方法 选取2017年1月1日~2019年6月30日在菏泽市第二人民医院诊断明确的40例剖宫产瘢痕妊娠(CSP)患者,按掷硬币法随机分为HIFU组(19例)和MTX组(21例)。HIFU组行HIFU联合清宫术治疗,MTX组行MTX联合清宫术治疗。比较两组术中阴道出血量、治疗后第3天β-HCG下降的情况、术后并发症(腹痛、发热、中转开腹)发生率、孕囊最大径、β-HCG降到正常时间及术后月经恢复正常时间。结果 两组术中阴道出血、治疗后第3天β-HCG下降情况、中转开腹发生率、β-HCG降到正常时间及月经恢复正常时间比较,差异有统计学意义(P<0.05);两组孕囊最大径、腹痛、发热发生率比较,差异无统计学意义(P>0.05)。结论 HIFU联合清宫术治疗剖宫产瘢痕妊娠血β-HCG降到正常时间短,术中阴道出血量少,中转开腹率低,月经恢复时间短,是一种安全无创技术。
Abstract:
Objective To analyze the efficacy of high-intensity focused ultrasound combined with uterine cervix in the treatment of cesarean scar pregnancy.Methods 40 patients with cesarean section scar pregnancy (CSP) diagnosed at Heze Second People’s Hospital from January 1, 2017 to June 30, 2019.According to the coin toss method, they were randomly divided into HIFU group (19 cases) and MTX group (21 cases). The HIFU group was treated with HIFU and uterine tidal surgery, and the MTX group was treated with MTX and uterine tidal surgery. Compare the amount of vaginal bleeding during the operation, the decrease of β-HCG on the 3rd day after treatment, the incidence of postoperative complications (abdominal pain, fever, transition to open abdomen), the maximum diameter of the gestational sac, the time when β-HCG decreased to normal, and the menstrual period returned to normal after surgery.Results Comparison of vaginal bleeding during surgery, the decrease of β-HCG on the 3rd day after treatment, the incidence of transition to open surgery, the time when β-HCG decreased to normal, and the time when menstruation returned to normal,the difference was statistically significant (P<0.05);Comparison of the incidence of abdominal pain and fever,the difference was not statistically significant(P>0.05).Conclusion HIFU combined with uterine hysterectomy is a safe and non-invasive technique for treating cesarean section scar pregnancy blood β-HCG to normal time, reducing the amount of vaginal bleeding during operation, low conversion rate, and short menstrual recovery time.
