摘要:
目的 观察阿加曲班联合阿替普酶静脉溶栓治疗急性脑梗死的临床疗效及对患者脑血管储备功能的影响。方法 选取2020年6月-2021年6月在我院诊治的84例急性脑梗死患者为研究对象,采用随机数字表法分为对照组和观察组,各42例。对照组采用阿替普酶治疗,观察组在对照组基础上采用阿加曲班治疗,比较两组临床效果、大脑中动脉血流动力学指标、脑血管储备功能(CVR)、神经功能缺损评分(NIHSS)、日常生活能力评分(ADL)及不良反应发生情况。结果 观察组治疗总有效率为95.24%,高于对照组的83.33%,差异有统计学意义(P<0.05);观察组舒张末期流速(Vd)、收缩期峰值流速(Vp)、平均流速(Vm)均大于对照组,差异有统计学意义(P<0.05);两组CVR均大于治疗前,且观察组大于对照组,差异有统计学意义(P<0.05);两组NIHSS评分低于治疗前,ADL评分高于治疗前,且观察组NIHSS评分低于对照组,ADL评分高于对照组,差异有统计学意义(P<0.05);观察组不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论 阿加曲班联合阿替普酶静脉溶栓治疗急性脑梗死效果理想,可增加患者脑血管储备功能,降低神经功能缺损,提高患者日常生活能力,不良反应少。
Abstract:
Objective To observe the clinical efficacy of argatroban combined with intravenous thrombolysis with alteplase in the treatment of acute cerebral infarction and its influence on cerebrovascular reserve function.Methods A total of 84 patients with acute cerebral infarction who were treated in our hospital from June 2020 to June 2021 were selected as the research objects. They were divided into control group and observation group by random number table method, with 42 cases in each group. The control group was treated with alteplase, and the observation group was treated with argatroban on the basis of the control group. The clinical effect, hemodynamic indexes of middle cerebral artery, cerebrovascular reserve function (CVR), neurological deficit score (NIHSS), daily living ability score (ADL) and adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 95.24%, which was higher than 83.33% of the control group, and the difference was statistically significant (P<0.05). The end-diastolic velocity (Vd), peak systolic velocity (Vp) and average velocity (Vm) in the observation group were higher than those in the control group, and the differences were statistically significant (P<0.05). The CVR of the two groups was greater than that before treatment, and the observation group was greater than the control group, the difference was statistically significant (P<0.05). NIHSS score of the two groups was lower than that before treatment, ADL score was higher than that before treatment, NIHSS score of the observation group was lower than that of the control group, ADL score was higher than that of the control group, and the difference was statistically significant (P<0.05). The incidence of adverse reactions in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05).Conclusion The effect of argatroban combined with alteplase intravenous thrombolysis in the treatment of acute cerebral infarction is ideal, which can increase the cerebrovascular reserve function, reduce the neurological deficit, improve the daily living ability, with less adverse reactions.
