摘要:
目的 分析尼可地尔联合卡维地洛对冠心病（CHD）心肌缺血患者心率及血压水平的影响。方法 选取2020年1月-2021年7月江西省万安县人民医院收治的66例CHD心肌缺血患者，按照随机数字表法进行分组，对照组（33例）行常规药物治疗，观察组（33例）在其基础上加用尼可地尔联合卡维地洛治疗，比较两组心电图检查结果（24 h ST段压低次数及时间）、心绞痛发作情况（发作频率、发作持续时间）、心率、血压（收缩压、舒张压）、心功能指标[左心室射血分数（LVEF）、血浆脑钠肽（BNP）、心肌肌钙蛋白I（cTnI）]。结果 两组心电图24 h ST段压低次数及时间均较治疗前减少，且观察组低于对照组（P＜0.05）；观察组治疗后心绞痛发作频率及持续时间低于对照组（P＜0.05）；两组治疗后心率、收缩压、舒张压均有下降，且观察组低于对照组（P＜0.05）；两组治疗后LVEF升高，BNP、cTnI水平下降，且观察组LVEF高于对照组，BNP、cTnI低于对照组（P＜0.05）。结论 尼可地尔联合卡维地洛对心肌缺血具有积极改善作用，可有效缓解CHD心肌缺血患者的心绞痛症状，控制其心率、血压水平，促进心功能改善。
Abstract:
Objective To analyze the effect of nicorandil combined with carvedilol on heart rate and blood pressure in patients with myocardial ischemia of coronary heart disease (CHD).Methods A total of 66 cases of CHD myocardial ischemia admitted to Wan’an County People ’s Hospital of Jiangxi Province from January 2020 to July 2021 were selected and grouped according to the random number table method. The control group (33 cases) was treated with conventional drugs, and the observation group (33 cases) was treated with nicorandil combined with carvedilol on the basis of the control group. The electrocardiogram examination results (24 h ST segment depression frequency and time), angina pectoris attack (attack frequency, attack duration), heart rate, blood pressure (systolic blood pressure, diastolic blood pressure), cardiac function indexes [left ventricular ejection fraction (LVEF), plasma brain natriuretic peptide (BNP), cardiac troponin I (cTnI)] were compared between the two groups.Results The number and time of ST segment depression in the two groups were lower than those before treatment, and the observation group was lower than the control group (P<0.05). The frequency and duration of angina pectoris in the observation group were lower than those in the control group (P<0.05). After treatment, the heart rate, systolic blood pressure and diastolic blood pressure of the two groups decreased, and those in the observation group were lower than the control group (P<0.05). After treatment, LVEF increased, BNP and cTnI levels decreased in the two groups, and LVEF in the observation group was higher than that in the control group, BNP and cTnI were lower than those in the control group (P<0.05).Conclusion Nicorandil combined with carvedilol has a positive effect on myocardial ischemia, which can effectively alleviate the symptoms of angina pectoris in patients with CHD myocardial ischemia, control their heart rate and blood pressure levels, and promote the improvement of cardiac function.
