摘要:
目的 应用Logit回归分析2型糖尿病患者(T2DM)微血管并发症的危险因素,探究其相关性。方法 筛选2018年1月-2020年12月于克拉玛依市中西医结合医院住院的T2DM患者1574例,对性别、年龄、吸烟、高血脂病史等进行Logit模型多元回归分析,评价影响T2DM微血管并发症相关3种疾病:糖尿病视网膜病变、糖尿病周围神经病变、临床蛋白尿的危险因素。结果 Logit回归分析结果显示,糖尿病视网膜病变与年龄、糖尿病病程、糖尿病治疗情况、脂肪肝等因素相关(P<0.05);糖尿病周围神经病变与年龄、吸烟史、高脂血病史、日吸烟量、空腹血糖、餐后2 h血糖、糖化血红蛋白(HbA1c)、每月酒精量、糖尿病治疗情况、高血压治疗情况等因素相关(P<0.05);临床蛋白尿与高脂血病史、糖尿病病程、低密度脂蛋白(LDL)、收缩压、糖尿病治疗情况、脂肪肝、高血压治疗情况等因素相关(P<0.05)。结论 T2DM微血管并发症与多种危险因素呈现相关性,重视微血管病变,减少相关危险因素,加强T2DM患者并发症的防治和管理,可有效改善患者预后,提高其生活质量。
Abstract:
Objective To analyze the risk factors of microvascular complications in patients with type 2 diabetes mellitus (T2DM) by Logit regression, and to explore its correlation. Methods A total of 1574 patients with T2DM who were hospitalized in Karamay Municipal Hospital of Integrated Chinese and Western Medicine from January 2018 to December 2020 were screened. Logit model multiple regression analysis was performed on gender, age, smoking, and history of hyperlipidemia to evaluate the risk factors of three diseases related to microvascular complications of T2DM: diabetic retinopathy, diabetic peripheral neuropathy, and clinical proteinuria. Results The results of Logit regression analysis showed that diabetic retinopathy was related to age, duration of diabetes, treatment of diabetes, fatty liver and other factors (P<0.05). Diabetic peripheral neuropathy was associated with age, smoking history, history of hyperlipidemia, daily smoking, fasting blood glucose, 2 h postprandial blood glucose, glycosylated hemoglobin (HbA1c), monthly alcohol consumption, diabetes treatment, hypertension treatment and other factors (P<0.05). Clinical proteinuria was associated with history of hyperlipidemia, duration of diabetes, low density lipoprotein (LDL), systolic blood pressure, treatment of diabetes, fatty liver, treatment of hypertension and other factors (P<0.05). Conclusion Microvascular complications of T2DM are correlated with a variety of risk factors. Paying attention to microvascular lesions, reducing related risk factors, and strengthening the prevention and management of complications in T2DM patients can effectively improve the prognosis of patients and improve their quality of life.
