目的 研究多囊卵巢综合征(PCOS)患者的糖代谢特征。方法 选择2014年1月~2018年3月我院收治的符合鹿特丹标准、未经治疗的PCOS患者935例,测定身高、体重,并行口服葡萄糖耐量试验(OGTT)和胰岛素释放试验(IRT),计算体重指数(BMI)、胰岛素抵抗稳态指数模型(HOMA-IR)、β细胞功能指数(HOMA-β)等参数。根据血糖水平将 PCOS 患者分为糖代谢正常(NGT)组、空腹血糖受损(IFG)组、糖耐量受损(IGT)组、混合型糖耐量受损(CGI)组及糖尿病(T2DM)组;根据体重指数
Abstract:
Objective To study the characteristics of glucose metabolism in patients with polycystic ovarian syndrome (PCOS).Methods From January 2014 to March 2018, 935 patients with Rotterdam-compliant and untreated PCOS were enrolled in our hospital. The height, weight, and oral glucose tolerance test (OGTT) and insulin release test (IRT) were calculated. Body mass index (BMI), insulin resistance homeostasis index model (HOMA-IR), β cell function index (HOMA-β) and other parameters. According to blood glucose levels, PCOS patients were divided into normal glucose metabolism (NGT) group, impaired fasting glucose (IFG) group, impaired glucose tolerance (IGT) group, mixed glucose tolerance (CGI) group and diabetes (T2DM) group. According to body mass index (BMI), it was divided into obese group and non-obese group; according to age, it was divided into puberty group and childbearing age group, and the differences of glucose metabolism in each group were compared.Results ①The incidence of IR in PCOS patients was 58.40% (n=546); the abnormal rate of glucose metabolism was 38.93% (n=364), of which IFG group accounted for 5.13%, IGT group accounted for 23.64%, CGI group accounted for 5.24%, T2DM group accounted for 4.92%.②The age, BMI, FPG, G60, G120, G180, HOMA-IR and HOMA-β of the PCOS obese group were higher than those of the non-obese group (P<0.01).③The age, FPG, G60 and G120 of the 3PCOS childbearing age group were higher than those of the puberty group (P<0.01). There was no significant difference in the BMI, G180 and HOMA-IR between the two groups (P>0.05).Conclusion The incidence of abnormal glucose metabolism is higher in PCOS patients, and the abnormal glucose metabolism is more serious in obese PCOS patients. Adolescence is characterized by insulin resistance and hyperinsulinemia, and the incidence of abnormal glucose metabolism increases with age.
