摘要:
目的 观察化瘀通痹方联合甲氨蝶呤治疗难治性类风湿性关节炎的临床效果。方法 选取2018年4月~2019年4月在我院治疗的70例难治性类风湿关节炎患者为研究对象，采用随机数字表法分为对照组和观察组，各35例。对照组采用甲氨蝶呤治疗，观察组在对照组基础上联合化瘀通痹方治疗，比较两组临床治疗总有效率、治疗后临床症状（关节僵硬时间、压痛评分、肿胀评分）改善情况、实验室指标（血沉、C反应蛋白、类风湿因子）以及临床不良反应发生情况。结果 观察组治疗总有效率（91.42%）与对照组（88.57%）比较，差异无统计学意义（P＞0.05）；治疗后观察组关节僵硬、压痛、肿胀评分均低于对照组（P＜0.05）；治疗后观察组血沉、C反应蛋白、类风湿因子均低于对照组（P＜0.05）；观察组临床不良反应发生率（8.57%）低于对照组（17.14%），差异有统计学意义（P＜0.05）。结论 化瘀通痹方联合甲氨蝶呤治疗难治性类风湿关节炎疗效与单纯甲氨蝶呤治疗疗效相当，但联合治疗可促进临床症状改善，减轻炎症反应，且临床不良反应少，可作为临床治疗的优选方案。
Abstract:
Objective To observe the clinical effect of Huayu Tongbi Recipe combined with methotrexate in the treatment of refractory rheumatoid arthritis. Methods A total of 70 patients with refractory rheumatoid arthritis who were treated in our hospital from April 2018 to April 2019 were selected as the research subjects, and the random number table method was used to divide them into control group and observation group,with 35 cases in each group. The control group was treated with methotrexate, and the observation group was combined with Huayu Tongbi Recipe on the basis of the control group. The total effective rate of clinical treatment, improvement of clinical symptoms (joint stiffness time, tenderness score, swelling score), laboratory indicators (erythrocyte sedimentation, C-reactive protein, rheumatoid factor) and clinical adverse reactions were compared between the two groups. Results The total effective rate of treatment in the observation group (91.42%) was not statistically significant compared with the control group (88.57%) (P>0.05).The scores of joint stiffness, tenderness, and swelling in the observation group were lower than those in the control group after treatment(P<0.05). The blood sedimentation, C-reactive protein, and rheumatoid factors in the observation group were lower than those in the control group after treatment(P<0.05); the incidence of clinical adverse reactions in the observation group (8.57%) was lower than that in the control group (17.14%) (P<0.05).Conclusion Huayu Tongbi Recipe combined with methotrexate treatment of refractory rheumatoid arthritis by methotrexate alone with comparable efficacy, but the combination therapy can facilitate clinical symptoms, reduce inflammation, and fewer clinical side effects, can be as a preferred embodiment clinical treatment.
