摘要:
目的 采用Logit回归模型分析2型糖尿病(T2DM)患者大血管并发症的危险因素。方法 筛选2018年1月-2020年12月于克拉玛依市中西医结合医院住院的T2DM患者1574例,对性别、年龄、吸烟、高血脂病史等进行回归分析,评价与T2DM大血管并发症相关5种疾病:冠心病、左心室肥厚、脑血管事件、下肢动脉硬化和颈动脉硬化的关系,总结影响T2DM大血管并发症的危险因素。结果 Logit回归分析结果显示,冠心病与年龄、高血压病程、糖尿病病程、低密度脂蛋白(LDL)、糖化血红蛋白(HBA1c)、糖尿病治疗情况、脂肪肝、高血压治疗情况相关(P<0.05);左心室肥厚与高血压病程相关(P<0.05);脑血管事件与性别、年龄、高血压病程、糖尿病病程、每月酒精量相关(P<0.05);下肢动脉硬化与性别、年龄、糖尿病病程、餐后2 h血糖相关(P<0.05);颈动脉硬化与性别、年龄、高血压病程、高血脂病史、收缩压、餐后2 h血糖、甘油三酯、维生素D相关(P<0.05)。结论 T2DM大血管并发症与多种危险因素相关,控制危险因素,降低并发症发生率,以确保T2DM患者有效提高生存率、改善生活质量、延长生存期。
Abstract:
Objective To analyze the risk factors of macrovascular complications in patients with type 2 diabetes mellitus (T2DM) by Logit regression model. Methods A total of 1574 patients with T2 DM who were hospitalized in Karamay Municipal Hospital of Integrated Traditional Chinese and Western Medicine from January 2018 to December 2020 were screened. Regression analysis was performed on gender, age, smoking, and history of hyperlipidemia. The relationship between five diseases related to T2DM macrovascular complications: coronary heart disease, left ventricular hypertrophy, cerebrovascular events, lower extremity arteriosclerosis, and carotid atherosclerosis was evaluated, and the risk factors affecting T2DM macrovascular complications were summarized. Results The results of Logit regression analysis showed that coronary heart disease was related to age, course of hypertension, course of diabetes, low density lipoprotein (LDL), glycosylated hemoglobin (HBA1c), treatment of diabetes, fatty liver and treatment of hypertension (P<0.05). Left ventricular hypertrophy was associated with the course of hypertension (P<0.05). Cerebrovascular events were related to gender, age, duration of hypertension, duration of diabetes, and monthly alcohol consumption (P<0.05). Lower extremity arteriosclerosis was associated with gender, age, duration of diabetes, and 2 h postprandial blood glucose (P<0.05). Carotid atherosclerosis was correlated with gender, age, course of hypertension, history of hyperlipidemia, systolic blood pressure, 2 h postprandial blood glucose, triglyceride and vitamin D (P<0.05). Conclusion T2DM macrovascular complications are related to a variety of risk factors. Controlling risk factors and reducing the incidence of complications can ensure that T2DM patients can effectively improve survival rate, improve quality of life and prolong the survival time.
