摘要:
目的 分析肌钙蛋白Ⅰ(cTnⅠ)与心肌酶谱联合检测在冠心病诊断中的临床意义。方法 将我院2020年6月-2021年6月共126例冠心病患者作为研究组,另选取同期126例健康体检者作为对照组,均接受cTnⅠ以及心肌酶谱检测,比较两组空腹血糖(FPG)、血脂[总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C)]、cTnⅠ、心肌酶谱水平[乳酸脱氢酶(LDH)、谷草转氨酶(AST)、磷酸肌酸激酶(CK)、α-羟丁酸脱氢酶(HBDH)、肌酸激酶同工酶(CK-MB)]以及cTnⅠ联合心肌酶谱诊断冠心病的效能。结果 研究组LDL-C、FPG高于对照组,差异有统计学意义(P<0.05);研究组TC、TG、HDL-C高于对照组,但差异无统计学意义(P>0.05);研究组cTnⅠ及心肌酶谱水平均高于对照组,差异有统计学意义(P<0.05);252例研究对象中,cTnⅠ检出阳性率48.41%,阴性率51.59%;心肌酶谱检出阳性率53.97%,阴性率46.03%;联合检测阳性率50.00%,阴性率50.00%;联合检测的灵敏度、特异度以及准确度均高于cTnⅠ与心肌酶谱单独检测,差异有统计学意义(P<0.05)。结论 肌钙蛋白与心肌酶谱联合检测对冠心病的诊断具有重要意义,能够有效弥补单一检测的缺陷,提高诊断准确性,从而指导临床治疗,改善患者预后。
Abstract:
Objective To analyze the clinical significance of combined detection of cardiac troponin Ⅰ(cTnⅠ) and myocardial enzymes in the diagnosis of coronary heart disease.Methods A total of 126 patients with coronary heart disease in our hospital from June 2020 to June 2021 were selected as the study group, and 126 healthy subjects in the same period were selected as the control group. The fasting plasma glucose (FPG), blood lipids [total cholesterol (TC), triglyceride (TG), low density lipoprotein (LDL-C), high density lipoprotein (HDL-C)], cTnⅠ, myocardial enzymes [lactate dehydrogenase (LDH), aspartate aminotransferase (AST), creatine kinase (CK), α-hydroxybutyrate dehydrogenase (HBDH), creatine kinase isoenzyme (CK-MB)] and the efficacy of cTnⅠ combined with myocardial enzymes in the diagnosis of coronary heart disease were compared between the two groups.Results LDL-C and FPG in the study group were higher than those in the control group, and the difference was statistically significant (P<0.05). TC, TG and HDL-C in the study group were higher than those in the control group, but the difference was not statistically significant (P>0.05). The levels of cTnⅠ and myocardial enzymes in the study group were higher than those in the control group, and the differences were statistically significant (P<0.05). Among the 252 subjects, the positive rate of cTnⅠ was 48.41%, and the negative rate was 51.59%. The positive rate of myocardial enzymes was 53.97%, and the negative rate was 46.03%. The positive rate of combined detection was 50.00%, and the negative rate was 50.00%. The sensitivity, specificity and accuracy of combined detection were higher than those of cTnI and myocardial enzymes alone, and the difference was statistically significant (P<0.05).Conclusion The combined detection of troponin and myocardial enzymes is of great significance in the diagnosis of coronary heart disease, which can effectively make up for the defects of single detection and improve the diagnostic accuracy, so as to guide clinical treatment and improve the prognosis of patients.
