摘要:
目的 探讨三酰甘油葡萄糖乘积指数(TyG指数)及LDL-C/HDL-C比率与2型糖尿病(T2DM)合并冠心病(CHD)的相关性。方法 选择2016年12月-2018年7月就诊于我院疑诊为CHD并行冠状动脉造影检查的T2DM患者387例,依据冠状动脉造影结果将确诊为CHD的患者设为研究组(283例),无CHD的患者设为对照组(104例),记录患者的临床资料、生化检测指标及冠状动脉影像学资料,并计算TyG指数、LDL-C/HDL-C比率及Gensini评分;比较两组临床资料、TyG指数、LDL-C/HDL-C比率,采用受试者工作特征曲线(ROC)评估TyG指数、LDL-C/HDL-C比率对T2DM合并CHD的诊断价值,并确定最佳截断值;Logistic回归分析T2DM合并CHD的危险因素;Spearman相关性分析TyG指数、LDL-C/HDL-C比率与Gensini评分的相关性。结果 研究组TyG指数、LDL-C/HDL-C比率均高于对照组(P<0.05);ROC曲线分析显示,TyG指数、LDL-C/HDL-C比率诊断T2DM合并CHD的曲线下面积(AUC)分别为0.646和0.651,当TyG指数、LDL-C/HDL-C比率分别取截断值7.170、2.080时,诊断T2DM合并CHD的效能最高,诊断的敏感度和特异度分别为44.50%、84.60%和55.20%、71.40%;Logistic回归分析显示,TyG指数≥7.170、LDL-C/HDL-C比率≥2.080是T2DM合并CHD的独立危险因素(P<0.05);Spearman相关性分析显示,TyG指数、LDL-C/HDL-C比率与Gensini评分呈正相关(r=0.114、0.152,P<0.05)。结论 TyG指数、LDL-C/HDL-C比率对诊断T2DM合并CHD具有提示意义,是T2DM合并CHD的独立危险因素,与T2DM患者的冠脉病变程度呈正相关。
Abstract:
Objective To investigate the correlation of triglyceride glucose product index (TyG index) and LDL-C/HDL-C ratio with type 2 diabetes mellitus (T2DM) complicated with coronary heart disease (CHD).Methods A total of 387 patients with T2DM who were suspected to be CHD and underwent coronary angiography in our hospital from December 2016 to July 2018 were selected. According to the results of coronary angiography, the patients diagnosed as CHD were set as the study group (283 cases), and the patients without CHD were set as the control group (104 cases). The clinical data, biochemical detection indexes and coronary artery imaging data of all patients were recorded, and the TyG index, LDL-C/HDL-C ratio and Gensini score were calculated. The clinical data, TyG index and LDL-C/HDL-C ratio were compared between the two groups. The receiver operating characteristic curve (ROC) was used to evaluate the diagnostic value of TyG index and LDL-C/HDL-C ratio for T2DM with CHD, and the optimal cutoff value was determined. Logistic regression analysis of T2DM with CHD risk factors; Spearman correlation analysis of TyG index, LDL-C/HDL-C ratio and Gensini score.Results The TyG index and LDL-C/HDL-C ratio in the study group were higher than those in the control group (P<0.05). ROC curve analysis showed that the area under the curve (AUC) of TyG index and LDL-C/HDL-C ratio in the diagnosis of T2DM combined with CHD were 0.646 and 0.651, respectively. When the cut-off values of TyG index and LDL-C/HDL-C ratio were 7.170 and 2.080, respectively, the diagnostic efficacy of T2DM combined with CHD was the highest, and the diagnostic sensitivity and specificity were 44.50%, 84.60% and 55.20%, 71.40%, respectively. Logistic regression analysis showed that TyG index≥7.170 and LDL-C/HDL-C ratio≥2.080 were independent risk factors for T2DM combined with CHD (P<0.05). Spearman correlation analysis showed that TyG index and LDL-C/HDL-C ratio were positively correlated with Gensini score (r=0.114, 0.152, P<0.05).Conclusion TyG index and LDL-C / HDL-C ratio are of significance for the diagnosis of T2DM complicated with CHD, which are independent risk factors for T2DM complicated with CHD and are positively correlated with the severity of coronary artery lesions in T2DM patients.
