摘要:
目的 探讨2型糖尿病并发慢性肾脏病患者外周血T淋巴细胞亚群变化与肾血管内皮功能障碍的相关性。方法 选取2018年1月-2020年2月我院收治的2型糖尿病并发慢性肾脏病患者110例作为研究1组,同时选取同时间段我院收治的单纯2型糖尿病患者110例作为研究2组,另外选取同时间段在我院体检的健康人群110例作为对照组。比较三组人群外周血T淋巴细胞亚群水平、肾血管内皮功能、FMD、分析2型糖尿病并发慢性肾脏病患者外周血T淋巴细胞亚群变化与肾血管内皮功能障碍的相关性。结果 研究1组CD3+、CD8+低于研究2组、对照组,CD4+、CD4+/CD8+高于研究2组和对照组,差异有统计学意义(P<0.05);研究1组ET-1、sICAM-1等肾血管内皮功能指标高于研究2组、对照组,FMD小于研究2组和对照组,差异有统计学意义(P<0.05);2型糖尿病并发慢性肾脏病患者CD3+、CD8+与肾血管内皮功能障碍发生呈负相关(r=-0.513,P<0.05,r=-0.645,P<0.05),CD4+、CD4+/CD8+高与肾血管内皮功能障碍发生呈正相关(r=0.428、0.511,P<0.05)。结论 在2型糖尿病并发慢性肾脏病的期间,外周血T淋巴细胞亚群发挥着重要的作用,同时外周血T淋巴细胞亚群变化与肾血管内皮功能障碍的发生存在明显的关系,可以根据分析结果对患者进行相应的治疗。
Abstract:
Objective To investigate the correlation between the changes of peripheral blood T lymphocyte subsets and renal vascular endothelial dysfunction in patients with type 2 diabetes mellitus complicated with chronic kidney disease.Methods A total of 110 patients with type 2 diabetes complicated with chronic kidney disease admitted to our hospital from January 2018 to February 2020 were selected as the study group 1, 110 patients with simple type 2 diabetes admitted to our hospital at the same time period were selected as the study group 2, 110 healthy people who underwent physical examination in our hospital at the same time period were selected as the control group. The levels of peripheral blood T lymphocyte subsets, renal vascular endothelial function, FMD were compared among the three groups. The correlation between the changes of peripheral blood T lymphocyte subsets and renal vascular endothelial dysfunction in patients with type 2 diabetes mellitus complicated with chronic kidney disease was analyzed.Results CD3+ and CD8+ in study group 1 were lower than those in study group 2 and control group, CD4+ and CD4+/CD8+ were higher than those in study group 2 and control group, the difference was statistically significant (P<0.05). The renal vascular endothelial function indexes such as ET-1 and sICAM-1 in study group 1 were higher than those in study group 2 and control group, and FMD was lower than that in study group 2 and control group, the difference was statistically significant (P<0.05). CD3+ and CD8+ in patients with type 2 diabetes complicated with chronic kidney disease were negatively correlated with the occurrence of renal vascular endothelial dysfunction (r=-0.513,P<0.05,r=-0.645,P<0.05). The high levels of CD4+ and CD4+/CD8+ were positively correlated with the occurrence of renal vascular endothelial dysfunction (r=0.428,0.511,P<0.05).Conclusion During the period of type 2 diabetes complicated with chronic kidney disease, peripheral blood T lymphocyte subsets play an important role. At the same time, there is a significant relationship between the changes of peripheral blood T lymphocyte subsets and the occurrence of renal vascular endothelial dysfunction, which can be treated according to the analysis results.
