摘要:
目的 采用文献计量学方法探讨血府逐瘀汤治疗心力衰竭的研究现状与特征。方法 以“血府逐瘀汤”“心力衰竭”“心衰”为检索词，检索中国知网（CNKI）、维普（VIP）和万方3大数据库，检索时间区间为建库至2022年5月31日，收集整理血府逐瘀汤治疗心力衰竭的相关期刊文献及学位论文，通过文献计量学方法对相关文献的年代、类型、期刊、机构等整体情况，以及临床研究特征进行分析。结果 ①文献整体情况：共收集文献139篇，发文量自2015年始明显上升；其中，期刊论文135篇，且临床对照试验研究128篇，明显多于基础性研究；发文机构为医疗机构的文献共计122篇，占87.77%，其中，以省级、地（区）级医疗机构为最多；核心期刊文献6篇、基金支持文献16篇，数量相对较少；②临床研究特征：96.88%的临床研究对象为慢性心力衰竭患者，心功能受损程度为轻中度；原发病多为冠心病，占研究总数的68.75%；有87.50%的临床研究将血府逐瘀汤作为西药的辅助治疗；约60.16%的临床研究联合使用了真武汤；具体处方中生地、赤芍为减掉频率最多的药物，黄芪为加入频率最多的药物。结论 血府逐瘀汤在治疗心力衰竭具有一定的优势及有效性。但整体文献质量相对较低，基础性、多角度研究相对匮乏，建议改进研究方法，增加研究的深度与广度，以期更有效的指导临床。
Abstract:
Objective To explore the current situation and characteristics of Xuefu Zhuyu decoction in the treatment of heart failure.Methods Taking "Xuefu Zhuyu decoction" "heart failure" and "heart failure" as the key words, three major databases of CNKI, VIP and Wanfang were searched. The retrieval time interval was from the establishment of the database to May 31, 2022. Relevant journal articles and dissertations on the treatment of heart failure with Xuefu Zhuyu decoction were collected and sorted out. The overall situation of the age, type, journal and institution of the relevant literature and the characteristics of clinical research were analyzed by bibliometrics.Results ①Overall literature: a total of 139 articles were collected, and the number of articles published increased significantly since 2015. Among them, there were 135 journal articles and 128 clinical controlled trials, which were significantly more than basic research. A total of 122 literatures were published by medical institutions, accounting for 87.77%, of which provincial and prefecture (district) medical institutions were the most; there were 6 core journal articles and 16 fund-supported articles, with a relatively small number; ②Clinical research characteristics: 96.88% of the clinical research subjects were patients with chronic heart failure, and the degree of cardiac function damage was mild to moderate; the primary disease was mostly coronary heart disease, accounting for 68.75% of the total number of studies; there were 87.50% of clinical studies using Xuefu Zhuyu decoction as an adjuvant therapy for western medicine; about 60.16% of the clinical studies combined Zhenwu decoction ; among the specific prescriptions, Shengdi and Chishao were the most frequently reduced drugs, and Huangqi was the most frequently added drug.Conclusion Xuefu Zhuyu decoction has certain advantages and effectiveness in the treatment of heart failure. However, the overall literature quality is relatively low, and basic and multi-angle research is relatively scarce. It is suggested to improve research methods and increase the depth and breadth of research in order to guide clinical practice more effectively.
